By clicking "Accept," and booking your trial class you acknowledge and agree to the following terms and conditions outlined in this waiver. This waiver is a binding legal document, so please read it carefully before proceeding.

I understand that participating in physical activities and exercises at Warrior Sports and Fun-X-tional Fitness (referred to as "the facility") involves certain risks and potential hazards. In consideration of being allowed to participate in the trial class offered by the facility, I hereby acknowledge and accept the following:

1. Assumption of Risk: I am aware that physical activities, exercises, and the use of fitness equipment involve inherent risks, including but not limited to the risk of bodily injury, strains, sprains, fractures, and in extreme cases, death. I voluntarily assume all risks associated with participating in the trial class.

2. Health and Medical Condition: I certify that I am in good physical condition and have no medical conditions that would prevent me from safely participating in physical activities. I have consulted with a medical professional if I have any concerns about my health and ability to engage in physical exercises.

3. Release of Liability: I hereby release, waive, and discharge Warrior Sports and Fun-X-tional Fitness, its owners, instructors, employees, and any affiliated parties from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may occur as a result of my participation in the trial class.

4. Indemnification: I agree to indemnify and hold harmless Warrior Sports and Fun-X-tional Fitness and its affiliates from any and all claims, liabilities, and expenses (including legal fees) arising out of or related to my participation in the trial class.

5. Use of Likeness: I grant Warrior Sports and Fun-X-tional Fitness the right to use photographs, video recordings, or other media of me taken during the trial class for promotional and marketing purposes without compensation.

6. Parent/Guardian Consent (if applicable): If the participant is a minor, I certify that I am the parent or legal guardian, and I hereby provide my consent for the minor to participate in the trial class. I agree to all terms on behalf of the minor.
